Above is a translation invariant surface with two parallel annular ends and two periodic Enneper ends (only the front one is visible). The quotient surface has genus 1.
It arises as the dihedralization limit of the symmetrized torus with two Enneper ends. As such it comes in a 1-parameter family and an easy to solve period problem. Below are two views of a surface with a different parameter value.
